Services May Include
Psychologist-Led Functional Capacity Assessment
Assessment of functioning across relevant areas of everyday life, including communication, social interaction, learning, self-management, self-care, domestic activities and community participation
Diagnostic Assessment or Review
Assessment to clarify or review autism, ADHD, intellectual disability, cognitive functioning or complex mental health presentations when this is clinically appropriate and relevant to support planning.
Intellectual Disability and Adaptive Functioning Review
Assessment of cognitive and adaptive functioning to clarify diagnosis, current support needs and the effect of disability on daily life.
NDIS Access and Plan-Review Reports
Psychological evidence describing diagnosis, permanence where supportable, functional impact, current supports, risks and clinically justified recommendations.
Our NDIS Report Approach
Our reports bring together clinical assessment, standardised findings, lived experience, available documentation and collateral information. Recommendations are individualised and linked to the person’s functional needs and goals.
Where needs relate primarily to physical function, assistive technology, equipment, home modifications or environmental access, an occupational therapy assessment may also be required. We will explain the boundaries of our assessment and recommend multidisciplinary input where appropriate.
Collaboration
With the participant’s consent, Zephyr may collaborate with support coordinators, recovery coaches, GPs, psychiatrists, occupational therapists, speech pathologists, behaviour support practitioners, educators and other members of the support team.
We welcome relevant reports before assessment so that existing evidence can be considered and unnecessary duplication reduced.
Zephyr is an NDIS provider that can
only accept self-managed and plan-managed funding. Service suitability and available funding should be confirmed before assessment begins...
