1. Initial Enquiry
Tell us who the assessment is for, the concerns involved and the questions you would like answered.
2. Assessment Planning
We consider the referral question and explain the proposed process, likely appointments, information required, fees and anticipated timeframe.
3. Understanding the Whole Person
Assessment may include interviews, standardised measures, observations, document review and collateral information. We consider strengths, history, health, relationships, environment and daily functioning alongside symptoms.
4. Clinical Integration
The available evidence is carefully brought together to form a coherent understanding of the person’s presentation, diagnoses and support needs.
5. Report and Feedback
Findings are explained in clear language. Where included in the agreed service, a written report provides the assessment outcome, formulation, functional implications and practical recommendations.
6. The Pathway Forward
Where appropriate, follow-up appointments can help the client or family understand the findings, prioritise recommendations and connect with relevant supports.
